Children 0-17, $49.00/month
Adults 18-59, $69.00/month
Adults 60+, $89.00/month
Family of Four, $200/month, each additional child $25.00/month
Membership includes;
2 visits per month (telemedicine or in clinic)
UNLIMITED access through patient portal
Same day Appointments
School/camp physicals
Discounted labs
10% discount for other clinic services including weight loss, B-12 injections, Cryoskin
Additional visits at discounted rate of $50 per visit
After hours telemedicine $75 per visit
After hours in clinic $150
Annual enrollment fee of $40.00 per patient. You may cancel at anytime with 30 day notice
Subject to $100.00 enrollment fee
In office medications and supplies may be subject to additional charge
